Follow patient changes

/v1/patients/base/changes

Patients changed since a cursor, oldest change first, inactive patients included. Use it to keep a copy of the register in another system.

Call with no since to read from the beginning, then with each answer's cursor. While hasMore is true, call again straight away. Changes from the last 30 seconds are held back until a later call.
