Publish a lab order to FHIR

/v1/investigations/lab/orders/:labOrderId/clinical/publish-fhir

Write a lab order's ServiceRequest, Observations and DiagnosticReport to the FHIR server configured for the deployment. Resources with an id are written with PUT, so publishing again updates rather than duplicates. Requires investigations.lab.release.
